Tax Exempt & Resale Certificate

If your business is exempt from sales tax — a reseller, a government body, a school, or a non-profit — send us your certificate here and we'll set your account up so tax isn't charged. We review these by hand, usually within one business day.

Already placed an order? Send your certificate within 48 hours of that order and we'll refund the sales tax you were charged on it. After 48 hours we're not able to refund that order, but your account will still be set up so tax isn't charged going forward.

Be signed in when you order. Your exemption is attached to your vbeltguys.com account, not to your company name — so you'll need to be logged in, using the same email address you give us below. Orders placed as a guest, or under a different email, will still be charged sales tax.

If you hold a multi-state or streamlined certificate, type Multi-state.
PDF or a clear photo. Up to 8 MB. Make sure the whole document is readable, including any signature and dates.
Certification

By submitting this form you certify that the certificate provided is true, correct, and currently valid, that you are authorized to submit it on behalf of the company named above, and that the purchases you make from V-Belt Guys / Midstate Industrial Supply qualify for the exemption claimed.

You agree to notify us promptly if the certificate is revoked, expires, or stops applying to your purchases. Typing your name below and submitting constitutes an electronic signature with the same effect as a handwritten one.
