If your business is exempt from sales tax — a reseller, a government body, a school, or a non-profit — send us your certificate here and we'll set your account up so tax isn't charged. We review these by hand, usually within one business day.

Please read this part. Your exemption is attached to your vbeltguys.com account, not to your company name. That means you must be signed in when you place an order, using the same email address you put on this form. Orders placed as a guest, or under a different email, will still be charged sales tax — and once an order is placed we can't remove the tax after the fact.

By submitting this form you certify that the certificate provided is true, correct, and currently valid, that you are authorized to submit it on behalf of the company named above, and that the purchases you make from V-Belt Guys / Midstate Industrial Supply qualify for the exemption claimed.

You agree to notify us promptly if the certificate is revoked, expires, or stops applying to your purchases. Typing your name below and submitting constitutes an electronic signature with the same effect as a handwritten one.
